Application of levobupivacaine combined butorphanol in labor analgesia during latent periodChinese Full Text

Abstract: OBJECTIVE To evaluate the analgesia effect of levobupivacaine combined with butorphanol used in latent phase of the first stage of labor and the impact on mother and infant.METHODS 60 full-term primiparas who had single birth,cephalic presentation,no counter-indication of vaginal delivery and intravertebral anesthesia,as the observation group（L group） that were implemented epidural labor analgesia when maternal cervical dilation was＜3 cm in latent phase.Another 60 primiparas were randomly selected as the control group（C group）without use of labor analgesia.The two groups were observed in vital signs,maternal labor pain（visual analogue scale,VAS）,motor nerve block（the modified Bronmge score）.Recorded labor time,use of oxytocin,mode of delivery,adverse effects,neonatal Apgar score and the results of umbilical arterial blood gas analysis.RESULTS Compared with analgesia before,L group had no statistical significance in hemodynamic indexes after analgesia（P＞0.05）,L group VAS scores after analgesia were lower（P＜0.05）.Compared with C group,L group increased incidence of use of oxytocin（P＜0.05）.There was no statistical significance between the two groups in labor time,motor nerve block,mode of delivery,adverse effects,neonatal Apgar score and the results of umbilical arterial blood gas analysis（P＞ 0.05）.CONCLUSION 0.1%levobupivacaine combined with0.004%butorphanol for labor analgesia during latent period pro-vides good analgesia,does not prolong the duration of labor,not increase the rate of cesarean section or instrumental delivery,has little influence on mother and infant.
